Winter Sport Physicals
Sports physicals are confirmed with Dr. Meese for Wednesday, November 17th from 9:00am to 11:00am. He will start at the high school then go to the middle school.
All Medical Forms can be downloaded off of our web-site cliffsidepark.edu
Completed and signed Parental Permission Form and completed and signed Medical History Report must be returned to Nurse no later than Friday, October 29th.
If a student uses his/her own physician; they must utilize only those forms provided by the NJSIAA
which can be downloaded off of our
web-site cliffsidepark.edu or picked up in the nurses office.
